I certify that all of the above information is true and accurate and I authorize the representatives of MVC FORSTERS to contact those listed above. I understand that if I adopt a cat/kitten from the MVC FOSTERS and it is later discovered that I have falsified any information on this application, the adoption will be null and void and I must return the cat/kitten(s) to the MVC FOSTERS.*
Vet Care Agreement*
I will return to Modern Vet Care or another veterinarian to complete any vaccines or spay/neuter when appropriate. I understand Modern Vet Care has performed an exam on the animal at the time of adoption and the animal appears to be in good health, but that health issues can arise at any time. I agree to follow up with Modern Vet Care or another veterinarian regarding any health concerns.*
